(Publisher): Introdcue a new flag,
_was_destroyed, to indicate whether an publisher instance has
already been destroyed.
(Publisher.Unsubscribe): Only disconnect if the publisher has not
been destroyed yet.
(Publisher.Destroy): Set the _was_destroyed flag to true.
